The information that you are asked to provide are the required information for the application of occupational permit, such as but not limited to the following:

1. Name, contact details, address, birthdate, gender, civil status, place of birth;
2. Employers business id, job description;
3. Copy of certificate of employment, heath certificate, and official receipt.
